Facts in brief:
On 28 March 2013, an employee of the company suffered serious harm while carrying out a welding job to strengthen the framework of a “lean-to” roof at the front of the workshop building. For reasons unrelated to the company, the employee chose to perform the task from an uncertified and unsecured cage held by a forklift. The employee’s weight tilted the cage to the side, and the cage was dislodged from the forklift and fell to the floor. The employee managed to remain inside the cage, but two of his fingers got crushed on the way down, probably as a result of the falling cage hitting a pole or some other equipment before hitting the ground.
