Notifying WorkSafe of fatalities in the workplace allows us to immediately investigate or follow up on events that caused the fatality.
This, along with claim information received from ACC, provides intelligence to facilitate our efforts to reduce the rate of work-related injuries occurring around the country.

ACC (SWIFT) fatalities to 31 August 2018
The information in this report comes from two sources:
1. WorkSafe New Zealand Fatalities Register:
- Contains information on confirmed fatal work-related incidents reported to WorkSafe.
2. System for Work-related Injury Forecasting and Targeting (SWIFT) fatalities:
- Contains fatalities information from SWIFT which combines data on ACC’s compensated fatalities and WorkSafe’s fatalities register.
Table 1. WorkSafe Fatalities:
- This table provides counts of confirmed workplace fatalities by year, month, industry level 1 and 2, region and age.
- Confirmed fatal work-related incidents reported to WorkSafe do not include fatalities that occurred outside of WorkSafe’s jurisdiction (e.g. maritime or aviation related fatalities, natural causes etc.).

- This table provides more detailed breakdowns of the data in Table 1, by detailed industry, region, age, focus area.
- This table includes an indicator to determine whether the fatality was related to quad bikes.
Table 3. SWIFT Fatality Rate:
- This table provides the three year rolling average SWIFT fatality rates, per 100,000 full-time equivalent employees (FTEs)
- Statistics New Zealand Household Labour Force Survey (HLFS) FTE counts are used as the denominator for these rates.
- This table includes the average fatality rate (Baseline Average) for 2008-2010, excluding the 29 workers killed in the Pike River Coal Mine Tragedy.
- This table includes the fatality target rates for both 2016 and 2020.

- This table provides counts of SWIFT fatalities by industry level 1 and 2, and region.
Table 5. SWIFT Fatalities Detail:
- This table provides count of SWIFT fatalities over the past five year period, by detailed industry, mechanism of injury, and region.
- SWIFT draws together data on the work related fatalities notified to WorkSafe and approved ACC work related fatality claims (which can include those from Maritime New Zealand, the Civil Aviation Authority, and New Zealand Police jurisdictions that resulted in ACC fatal claims). It excludes fatalities involving bystanders, people under the age of 15, gradual process injuries, and work-related disease.
- More recent ACC data takes time to stabilise. WorkSafe recommends applying a three month lag to the SWIFT fatality figures.
- SWIFT fatality data will differ from counts of fatalities occurring within WorkSafe's jurisdiction as WorkSafe counts include bystanders and exclude maritime, aviation, and many transport related fatalities.
- While SWIFT draws on ACC data, differences in counting criteria mean it may not match ACC figures, and should not be considered official ACC data.
- HLFS full-time equivalent employee counts are the sum of the count of all full time workers plus half the count of part time workers.
- A SWIFT fatality may have multiple mechanisms of injury.
- Other fatalities may not be able to be classified against a mechanism. Therefore, fatalities by mechanism should not be used as an overall count of fatalities.
- WorkSafe has changed the way it reports fatalities in the Forestry and Agriculture sectors to align with the Australia and New Zealand Standard Industry Classification. Some fatality data has moved into ‘Forestry Support Services’ and ‘Agriculture and Fishing Support Services’ which will reduce the numbers recorded in the Agriculture and Forestry industries (see AFF2017 variable). Taken overall the sectors’ performances have not changed – the total burden of Forestry and Agriculture fatalities remain unchanged.
- Data quality has improved over time so caution is advised when interpreting the data
